KEEP
UP-TO-DATE WITH CHANGES AT THE PRACTICE
We are
constantly trying to improve our service to patients and
are currently looking into ways that we can inform you of any changes
that we make at the surgery.
Currently
we put notices in the waiting room and information on our website,
but often people do not find out about changes until they need a
doctor.
We
would like to improve this system by sending emails to patients.
If
you are willing to receive notification of changes and general information
from the surgery, please:
- Log
into Emis Access and click on View/Update
your contact Info. You can add your email address in
and submit the information to us.
Or

Email
the surgery at dco-pct.reception@nhs.net and
put the subject as 'Email Updates'. Please
also include your name and address, so that we
can enter this onto your computerised records.
We
will not pass your details onto any third party.
Please
remember to keep the surgery informed of any changes to your personal
details including, telephone numbers, email address etc.
