can email the completed form to:
dco-pct.reception@nhs.net
Please
ensure that you complete this form and return it to the surgery
at least three month prior to your holiday if possible. Please
note, if you book a last minute holiday, try and leave enough time
to get fully immunised.
Please
be aware private prescriptions for anti-malarials, yellow fever
and some other vaccinations carry a charge, please discuss this
with the nurse at your appointment.
